This is a Modem Daughter Card (MDC) which is installed in a CDC (Communications Daughter Card) slot.
This slot is sometimes also refered to as a MDC (Mobile Daughter Card) slot.

Linux driver
The Linux driver from SmartLink should work.
Latest version of this driver is 2.9.10 and supports both the 2.4 and 2.6 kernels
